I was looking for a medical form, I can get this done with my aviation medical later this month, hate wasting money on forms when I don't need to.

Race license medical every year over the age of 50 Nick

 

If you pitch it right you can make it last for two years

I have an aviation medical every year do not a problem there, hope my AME doesn't charge me too much for signing another bit if paper....
